MI Choice Medicaid Waiver Program
.wmf) - The Waiver Program is designed to be an alternative to nursing home care. Specifically, it allows the same services that are usually provided in a nursing home to be provided in a person’s own home instead. The official name of the program is the “Home and Community-Based Waiver for the Elderly and People with Disabilities.”
- There are 13 services available through the Medicaid Waiver Program. They include: home-delivered meals, transportation, private duty nursing, medical supplies and equipment, homemaker service, adult day care, respite care, chore services, counseling, training, supervision, emergency response systems and home modifications.
- Adults from 18 to 64 years of age who have disabilities significant enough to qualify for nursing home care and seniors age 65 and older who have multiple unmet needs are eligible for Medicaid.
- There is an initial prescreening done by DAAA Intake, followed by an assessment conducted by a registered nurse. A plan of care is developed with the active involvement of the participant and includes family and community resources.
- Services provided through the Medicaid Waiver Program are covered by Medicaid.
- The Detroit Area Agency on Aging provides Waiver services to residents in the following communities: Detroit, Hamtramck, Harper Woods, Highland Park, and the five Grosse Pointes.
- There are three major criteria to be used in evaluating your clients for possible eligibility:
- Clients must be on Medicaid or eligible for Medicaid.
- Clients must meet the medical eligibility for nursing care. The eligibility assessment is done by a RN.
- Clients must have multiple or complex needs that can be alleviated through the provision of a Waiver service.
- Income guidelines: $1,635 per person per month, and assets are limited to $2,000 per person in checking and/or savings accounts.
